Resident bathing in nursing homes is often considered intuitive. However, a 2023 study on bathing conducted in 8 nursing homes in Orange County, California (1), showed a lack of proper bathing routines. This includes inadequate cleansing, process failures such as not changing dirty wipes or cloths, and a lack of following a proper clean-to-dirty sequence (100% for bed bath and 96% for shower).

Ensure a proper washing sequence

A consistent washing sequence not only maintains hygiene but also provides a sense of structure and care for residents. By following an organized approach, you can help make the bathing process feel safe, comfortable, and respectful. This guide outlines practical steps to create a smooth and dignified bathing experience:

Step 1 – Prepare the environment for bathing

Before starting the bathing process, ensure the room is calm, clean, and warm. Gather all necessary supplies, such as towels, soap, wash gloves, and a basin or wipes. A well-prepared environment helps your resident feel more at ease and comfortable during the bath, creating a safer and smoother experience.

Step 2 – Follow a top-down washing sequence

To keep the bathing process thorough and gentle, follow this simple top-down sequence:

  • Begin with the face, then move down to the neck, chest, and shoulders.
  • Wash the arms and hands, followed by the belly.
  • Wash one leg at a time, starting from the thigh and moving down to the foot. Use a new cloth for each leg.
  • Wash the residents back, starting with the upper back and moving down to the lower back.
  • Wash the groin area with extra care and respect.
  • Lastly, clean the anal area, using a gentle approach to maintain comfort and dignity.

Step 3 – Use a fresh glove or wipe for each area

For hygiene and to prevent cross-contamination, always use a fresh wash glove, wipe, or cloth for each part of the body. Disposable dry wash gloves (used with soap and water or no-rinse cleansing foam) or wet wash gloves (containing the right amount of soap and water) are great options to keep things clean and simple.

Step 4 – Pay special attention to sensitive skin

Some residents may have delicate or irritated skin, such as those affected by Incontinence-Associated Dermatitis (IAD). For these areas, be extra mindful and use soothing, dermatologically tested products. Your careful and gentle approach helps prevent irritation and ensures that your resident feels comfortable and cared for throughout the bathing process.

Step 5 – Communicate and reassure

Throughout the bathing process, communicate with the resident to help them feel more involved and reassured. Explain what you're doing at each step, and ask for their preferences or concerns. This helps maintain their dignity and keeps the experience positive.
